How-To Prepare For Your Initial Evaluation
What to Know & What To Wear
- Arrive 15 minutes early. This will allow time to complete and review your registration paperwork. It will also give us time to answer any of your insurance questions.
- Wear comfortable, loose clothing. You’ll need to expose the body part that is being treated (knee, shoulder, back, etc.). Loose fitting apparel such as sweats and t-shirt are ideal.
What to Expect
- Be prepared to undergo a complete physical exam on your first visit. It may take between 45-60 minutes for the entire visit.
New Client Forms Process
New Client Forms are here to make the registration process as easy as possible. Please complete the Registration Packet, The Client/Therapist Game Plan and the Body Part Forms paperwork online and print. Please bring all forms on your first visit. Due to HIPPA regulations, completed forms will not be saved unless downloaded to your own computer.
Our client managers will verify your insurance benefits before your first visit so that you will know up front what your cost will be regarding copays and deductibles.

Treatments & Terminology
- Therapeutic Activity – specific set of exercises that reproduce specific daily or work activities for improvement.
- Neuromuscular re-education – specific set of exercises to improve balance & muscle recruitment for joint stability may include taping of body part for support or improved muscle reaction.
- Therapeutic Exercise – stretching and strengthening to improve mobility, flexibility & strength.
- Manual Therapy – specific hands-on and instrument assisted techniques performed by therapist to joints and muscles to improve mobility and decrease pain and spasm.
- Gait Training – improves walking, weight bearing and may include an assistive device.
- Laser Therapy – light energy accelerates and strengthens tissue repair, reduces pain.
- Inferential Electric Stimulation – mild tingling sensation, helps reduce pain and swelling.
- Spinal Decompression Traction – computer assisted table stretches spine to decrease disc pressure and relieve nerve root pain.
- Ultrasound – sound waves heat deep tissue to decrease pain, relax spasm, prevent adhesions.
- Home Exercise Program -performed by you to compliment your therapy between your sessions and after your discharge.
Payment/Insurances
Our client managers will verify your insurance benefits before your first visit so that you will know up front what your cost will be regarding copays and deductibles.
Clients who have a deductible that has not been met will need to pay $100 for the initial visit and $100 for each follow up visit covering the majority of the cost and preventing a large bill from being owed at the end. Copays will remain due at the time of service. You will be sent monthly statements and any additional charges not covered will need to be collected at that time. We don’t like this anymore than you do. This is happening to everyone.
The office will be open in discussing prices and costs with you. The last thing you want is an unexpected bill later. If you intend to use your insurance, call the number on your card and verify your deductible and co-payment before your first visit. We will be happy to work with you to establish a payment plan, if needed but remember-Your Insurance is Your Responsibility.
Always know:
Your Deductible (the amount of expenses that you will pay out-of-pocket BEFORE your insurance will any of the expense)
Your Co-Payment (the fixed amount that you must pay each visit which is not covered by your insurance)
