How-To Prepare For Your Initial Evaluation: What to Know & What To Wear

  • Arrive 15 minutes early.  This will allow time to complete and review your registration paperwork.  It will also give us time to answer any of your insurance questions.
  • Wear comfortable, loose clothing.  You’ll need to expose the body part that is being treated (knee, shoulder, back, etc.). Loose fitting apparel such as sweats and t-shirt are ideal.

We will be happy to work with you to establish a payment plan, if needed but remember-Your Insurance is Your Responsibility.

Your  Deductible (the amount of expenses that you will pay out-of-pocket BEFORE your insurance will any of the expense)

Your Co-Payment (the fixed amount that you must pay each visit which is not covered by your insurance)
